Diabetes is one of the most prevalent chronic diseases among America’s Health Center patients. Nearly six million patient-visits to a health center involve a patient with diabetes annually. Offering access to quality care, health centers are vital to helping communities address diabetes, a disease that impacts nearly 26 million children and adults in the United States.
