Know the 2024-2025 School Immunization requirements

Florida Department of Health  Contributor Florida state statute requires that prior to admittance to or attendance in a public school, private school, virtual school, or non-public school, each student shall provide the school with a Florida Certificate of Immunization Form DH 680, part A showing vaccine compliance from a licensed practicing physician or county health department.
